How to Register for ACADEMIC YEAR 2011-2012: |
Download Registration Form 
- Online: Print this form (requires Adobe Acrobat Reader
) and turn it in to the office.
- Phone: 206-242-1239 and reserve your spot with a credit card over the phone.
- Fax: Complete the Registration Form and fax your form to 206-242-8363.
- Mail: Complete the Registration Form and mail the form and fee payment to 15811 Ambaum Blvd SW, Suite 160, Burien, WA 98166
Advance payment of registration fee plus one month tuition holds your student’s place in class.
Registration Fee:
- $20 Registration fee per new student
- $15 Registration fee per returning student
- $40 max registration fee per family
- 1/2 off discount per child if tuition is setup on auto-pay

Tuition Discounts |
- Multi-Class Discount:
Discount may be used for group music lessons and/or dance classes. It applies to individual students, or multiple
students within a single household. Discount does not apply to private lessons, including duo and trio lessons.
5% off if a student takes 3-4 classes per week
10% off if a student takes 5-6 classes per week
15% off if a student takes 7 or more classes per week
- Advance Payment:
Half Year- pay tuition in two "half year" installments and receive 5% off each tuition installment plus
1/2 off the registration fee. September to January installment (first 5 months) is due by October 1, 2011. February to June installment
(second 5 months) is due by February 10, 2012. If you choose to cancel the second installment, your tuition will revert back to
monthly payments; which are due on the first of each month. Discount cannot be used in conjunction with the Multi‐class discount.
Full Year- Pay your entire annual tuition in full by October 10, 2011 and receive 5% off your entire
tuition amount, plus 10% off all performance fees, plus 1/2 off the registration fee. Discount cannot be used in conjunction with the
Multi-class discount.
- Auto-Pay Discount:
Set up your account for direct withdrawal payments (credit card or checking/savings account) and receive 1/2 off the registration fee per child.
- Home School:
If you are home schooling your children, you are eligible for a $5 discount off each group music or dance class taken
during the day. Discount may not be used in conjunction with the Multi‐class discount (tuition will be discounted according to
whichever is greater) or used during the summer or toward private lessons, including duo and trio lessons.
- Refer-a-Friend:
Refer a friend to Momentum Music and Dance Academy and receive $10 off one month's tuition.
- Scholarships:
Scholarships are available. An application must be filled out and reviewed by the studio owner. Tuition discount
is based on individual need.
- Trial Lessons:
Trial lessons are available. Trial lessons for group dance classes are free; however, trial lessons for private
lessons, including duos and trios, are charged the single lesson rate. Maximum number of free trial dance lessons is one.

How to Withdraw from Momentum |
There is a one month minimum tuition for all group lessons. One month's notice from the first of the month is required to discontinue any classes. Withdrawal takes effect on the 1st of the following month after the withdrawal form has been received. Withdrawal must be done at the school office and not with the teacher. To withdraw from lessons a parent or adult student must:
- Inform school administration, and
- Complete and sign a withdrawal form and turn it in to the office
All automatic bank debiting or credit card charges will stop after the one month notice period. Momentum Music & Dance Academy reserves the right to terminate lessons of any student without notice. In such a case, a refund for unused lessons will be given.
